Evaluation and Tracking Impact for H5N1

Two evaluations have been developed to help the Extension Foundation and EXCITE collect valuable data regarding the H5N1 efforts in your communities and states. By reporting and collecting evaluation data, you will enable us to tell the national impact story of what Extension is doing with H5N1.

We will be asking you to complete an evaluation each month on the types of activities and campaign products that you, as educators, are delivering to your clients. We also encourage you to share a brief, 4-question evaluation with your clients to evaluate their knowledge and additional resource needs.

  1. H5N1 Monthly Survey for Educators 

    • Survey Link

    • Purpose: To gather information about activities, campaign products, and additional resource needs.

  2. H5N1 Survey for Clients

    • Client Survey Link

    • Purpose: To assess clients’ knowledge of human and animal health, PPE usage, and their intent to utilize provided information.

Links and QR codes for these evaluations will also be posted on the H5N1 website.
